Senior Community Physiotherapist

1x fixed term contract for up to 12 months

Due to the current post holder commencing maternity leave, an exciting opportunity has emerged for an enthusiastic and skilled Physiotherapist to join the Integrated Community Team (ICT) in the Stroud District on a fixed term contract until March 2026.

The ICT consists of Community Nursing, Physiotherapy, Occupational Therapy and the Home First/Reablement Team. We work collaboratively with the wider MDT within our service and our Integrated Care System (ICS) partners to enable individuals to remain as healthy and independent as possible within their own homes.

This is a multifaceted role with a varied caseload incorporating both triage and urgent response of patients referred via the referral centre, and patients from the community waiting list. The post holder will also be responsible for the delegation of work to other members of the physiotherapy team within the ICT, as well as holding a caseload. This is an ideal opportunity for an existing Band 6 or experienced Band 5 looking for a new challenge who would like to work in new ways within the Stroud area.

We can offer you the opportunity to work in a very supportive integrated team, with access to training, regular supervision, annual appraisal and the autonomy to be a lead professional in our service delivery.

Main duties of the job

To work as a Band 6 Physiotherapist participating as a member of the community team and independently managing a specialised clinical caseload. You will also be required to manage and triage work within the Stroud Referral Centre on a rota basis.

The post-holder will be required to assess, formulate, evaluate and monitor specialised rehabilitation care packages which will involve complex decision making within the community, including Home First services and referral centre. The role can require urgent responses to priority cases, and will require negotiation and co-ordination of a number of multidisciplinary team members to deliver specialised rehabilitation care programmes.
