Job summary
We are seeking to employ a Mental Health Social Worker for 3 days a week to be part of our service and continue supporting the good work we are delivering in the borough of City and Hackney.
To provide a high quality and effective mental health social work service to the residents of the London Borough of Hackney. To respond to the needs of adults with mental disorder and their carers in a timely manner, within the boundaries of statutory legislation, and with regard to individual circumstances. To assess need using a person centred approach and effectively asses and manage risk. To plan and deliver outcome focused interventions linked to promoting independence & social inclusion. When appropriate act as Care Coordinator within the framework of the Care Programme Approach, taking responsibility for the provision of a social work service to mental health services users and carers in the community and in hospital.
Main duties of the job
Assessment
- To work with guidelines and policy of the CPA, Mental Health Act, Mental Capacity Act, housing and benefits legislation, Personalisation agenda and The Care Act 2014 to establish individual programmes of care.
- To undertake comprehensive risk assessments and devise/implement risk management plans appropriate to level of risk.
- If appropriate to team setting, conduct Carers Assessments in accordance with Carers (Recognition and Services) Act 1995 and to ensure service users and their carers participate as fully as possible in case discussions.
- To work in multi-disciplinary settings including participating in multi-disciplinary assessments, risk assessment and risk management plans.
- To carry out all reviews in line with the Promoting Independence commitment to ensure that services continue to meet assessed needs and provide value for money. The review will also respond to changing needs, risks and the wider community to ensure that the support plan remains appropriate and individuals are kept safe.
Care Planning, Accountability, and Service Delivery
- To provide duty cover, to assist in the prioritisation, allocation and support for referrals or in providing immediate professional guidance in crisis issues, arranging immediate support to manage risk and escalating issues of concern to more senior practitioners.

Experience

- Experience of working in a secondary mental health community team as a qualified member of staff implementing the recovery model of care.
- Experience of making full psychosocial assessments and writing reports.
- Experience of working as part of a multi-professional team
Desirable
- Experience of working in a multiracial inner city area
- Experience of working in a community team.
- Experience of providing support and education to carers/families and supporters
Knowledge
Essential
- Understanding of the Mental Health Act 1983 and Mental Capacity Act 2005
- Understanding of the principles and practice of CPA and the Care Act 2014.
- Understanding of the key themes of clinical governance and evidence based practice.
