The successful candidate will work across the 5 GP Practices in the Primary Care Network, supporting (or leading) projects ranging from medicines reconciliation post-discharge, medicine safety alert processes, daily medicine-related tasks, involvement in seasonal vaccination programmes and facilitating medication reviews.
We expect contribution to safe and effective prescribing across the PCN, acting on best practice from local and national guidelines.
Overarching goals are to help keep patients safe and well out of hospital and reduce the demands on general practice regards medicine use.
The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations processes, while addressing both the public health and social care needs of patients in the GP practices that make up the PCN.
The post holder will develop and encourage a positive culture of medicines optimisation principles to assist the PCN in achieving local and national medicines-related objectives.
Promotes high quality cost-effective & evidence-based use of medicines within the PCN.
Be a contributing team member to the PCN Pharmacy Team to respond to medication-related queries generated by patients, GPs, other health care professionals and staff.
Assist the PCN Pharmacy Team to develop and ensure a safe efficient medicines optimisation programme including repeat prescribing, medication review & medicines reconciliation services.
Be proficient with TPP/SystmOne GP prescribing system & supporting IT to record personally generated information & maintain a database of information relating to the work done in the practices.
Identify & produce recommendations for medication optimisation for clinical pharmacists, GPs & other multi-disciplinary team members.
Work with care home staff to improve safety of medicines ordering & administration if required.
Encourage good management of common/minor/self-limiting ailments, including promotion of self-care & homely remedies.
Liaise with all members of the local PCN including community pharmacies & community nursing team, secondary care providers & regional CCG teams on prescribing-related matters.
Aim for good collaborative working practices & develop relationships with other professionals across PCNs & the wider health & social care system.

The Chippenham, Corsham and Box Primary Care Network (CCB PCN) provides proactive and coordinated care across the local population of62,000 people.
We have a strong focus on health promotion and personalised care, supporting people to make informed decisions about their health and social care.
Situated a few miles south of the M4, near junction 17 Chippenham, the main work location is at the PCN Hub on New Road, but can be across the entire PCN area.
Minimum Requirements
The post-holder will take responsibility for their own development, learning and performance.
This is a role taking advantage of the NHS Network Contract DES for Additional Roles Reimbursement Scheme. Enrolment (or completion) of the CPPE Primary Care Pathway is expected.
Working towards being a pharmacist independentprescriber within 3 years.
Meet the GPhC CPD requirements for registration on an annual basis.
Complete any internal training required by the PCN.
Main Duties
Achieves efficiencies and effectiveness within the prescribing activities of the PCN group of GP practices.

Undertake Structured Medicine Reviews (SMRs) in line with Network Contract DES either face to face or via telephone support on identified stratified-risk patients that may include residents in care homes, in their own, with long-term conditions, and/or learning disabilities.
Be proficient with TPP/SystmOne GP prescribing system and supporting IT to record personally generated information and maintain a database of information relating to the work done in the practices.
Identify and produce recommendations for medication optimisation for clinical pharmacists, GPs and other multi-disciplinary team members.
Work with care home staff to improve safety of medicines ordering and administration if required.

Contribute to the safety of drug monitoring processes and drug alerts.
Analyse, interpret and present medicines data to highlight issues and risks to support decision making.
Advocate and implement local, regional and national formulary recommendations, including public health campaigns.
Liaise with all members of the local PCN including community pharmacies and community nursing team, secondary care providers and regional CCG teams on prescribing-related matters.
Aim for good collaborative working practices and develop relationships with other professionals across PCNs and the wider health and social care system.
Undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.
Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.
Respond to the changing NHS landscape, including contributing to Covid-19 vaccination programme, for example becoming a vaccinator and/or vaccine manager.
