We are looking to recruit a highly motivated clinician with excellent clinical reasoning skills to join the Community Recovery Service.The Community Recovery service is an integrated health and social care service for adults identified with uni and multi-disciplinary needs for recovery. The service will support independence by providing rehabilitation and reablement after an acute illness, injury or change in life circumstances. It aims to restore and maximise function or prevent deterioration in physical, psychological and social functioning through episodes of rehabilitation and reablement. Therapists provide support and rehabilitation in the home.The Community Recovery Service provides therapy for various pathways including:o Core/ general rehabilitation,o Discharge to Assesso Reablement (CRS+)o Integrated discharge hub at west Middlesex hospital.All health and social care professionals within the Community Recovery Service will have a set of core skills that all staff in the team are able to deliver regardless of profession. This supports the single assessment process, maximises use of resources and minimises duplication. In addition, team members will retain specialist skills and knowledge which will be utilised as required to support specific clinical pathways. Specialists will also support other team members' work to ensure fulfilment of relevant goals.
The successful candidate will act as a specialist clinician, participating as a member of the team while independently managing a specialist clinical caseload.o Manage and prioritise a professional caseload within the interdisciplinary service through efficient workload management, supervision and adjustment of service provision to meet changing priorities and ensure individual patients receive intervention timeously and appropriately.o They will provide high quality comprehensive physiotherapy assessment and treatment to adults referred to the Hounslow Community Recovery Service.o Develop and deliver an individualised treatment programmes based on a problem solving and advanced clinical reasoning approach to the patients referred and monitor, evaluate and modify treatment in order to measure progress and ensure effectiveness of intervention using valid outcome measures.o Work closely with the multi-agency team in order to meet the needs of the patients and enable them to receive a comprehensive service.o Undertake all aspects of physiotherapy care as an autonomous practitioner(with support from a senior Physiotherapist & Clinical Lead).
