Occupational Therapist

Job summary

Intermediate Care in the Central Manchester area is looking to recruit an innovative, enthusiastic Occupational Therapist, to be part of a lively and dynamic multi-professional team comprising health, social services and independent sector staff. Intermediate Care is a constantly evolving service so if you like change and challenges this is the job for you.

The service has beds in dedicated Intermediate Care units both nursing and residential and has capacity to deliver Intermediate Care to 60 - 70 people in their own homes. All staff are expected to rotate with in the service, so it is essential that applicants can travel independently within the local geographical area to meet the service needs.

You must have experience of working in the NHS and it would be desirable that you have experience of working in a rehabilitation/ community setting.

Applicants must have a recognised Occupational Therapy qualification and hold a current HCPC Registration.

Main duties of the job

The post holder will be responsible for providing specialist assessment and treatment for patients in the community who have been discharged from the acute sector, or have been identified as needing occupational therapy input as part of an integrated, multi-professional and multi-agency intermediate care service. The post holder will work with patients in their own homes and also within the Intermediate care bed base.

The post holder will follow agreed protocols for ensuring appropriate and timely input. They will also ensure that pathways of care and communications are maintained with all agencies working with the service

The post holder will work as a member of the Manchester Community Response Intermediate Care Team, ensuring individual needs are met, enabling choice, participation in rehabilitation and an improved quality of life for the patient.

The post holder will aim to prevent re-admission to hospital or inappropriate admission to residential or nursing homes by maximising function and independence of patients in their own homes and their community, whilst respecting patient choice and autonomy.
