Central Sutton PCN is looking to employ Dietician to work directly with its practices to improve the experience of patients of all ages with diet-related disorders on how they can improve their health and prevent disease by adopting healthier eating and drinking habits. The Dietician will undertake advanced health advice to patients to learn about their health goals, dietary needs, preferences and restrictions. This role will be working autonomously, but will have the support of a multi-professional primary care team made up of doctors, nurses, clinical pharmacists, physician associate, health coaches, social prescribers and paramedics. You will have strong experience and knowledge of working with different patient groups and needs across a diverse population. You will have excellent communication skills in addressing and providing health advice to patients on special dietary requirements including food allergies, coeliac disease, diabetes etc. as well as educating health professionals and the public about nutrition. Provide specialist nutrition and diet advice to patients, their carers, and healthcare professionals through treatment, education plans, and prescriptions. Educate patients with diet-related disorders on how they can improve their health and prevent disease by adopting healthier eating and drinking habits; Develop and deliver evidence-based nutritional intervention plans specifically for MASLD management Monitor patient progress through appropriate nutritional assessments and measurements Work collaboratively with health coaches to reinforce lifestyle changes and dietary modifications. Contribute to the development of educational materials and resources for patients with MASLD Work as part of a multi-disciplinary team to gain patients cooperation and understanding in following recommended dietary treatments; Develop, implement and evaluate a seamless nutrition support service across the PCN, working with community and secondary care where appropriate, and aimed at continuously improving standards of patient care and wider multi-disciplinary team working; Work with clinicians, multi-disciplinary team colleagues and external agencies to ensure the smooth transition of patients discharged from hospital back into primary care, so that they can continue their diet plan
Sutton has a population of approximately 200000 residents registered to 21 practices and there are currently 4 Primary Care Networks (PCNs); Carshalton, Cheam & South Sutton, Central Sutton and Wallington PCN; each serving a population of approximately 50000 patients. Primary care networks (PCNs) form a key building block of the NHS long-term plan.
