We are seeking a highly motivated and dedicated Network Pharmacist to join our Primary Care Network (PCN). As part of our team, you will play a crucial role in providing expert advice on medication management, medicines safety, conduct Structured Medication Reviews and supporting the broader healthcare team to improve patient outcomes. You will work closely with GPs, practice nurses, and other healthcare professionals within the Hadleigh Practice and wider PCN to deliver integrated, patient-centered care.
Key Responsibilities:
Poole North Primary Care Network have a team of Pharmacists and Pharmacy Technicians working from our Birchwood Hub but the role will also align with the Hadleigh Practice where you will join their team of two existing PCN pharmacists, one practice pharmacist and one pharmacy technician as well as supporting the "hub team" one day a week.
We are a friendly and supportive team achieving excellent results from financial stewardship programmes, BP@home work, CVD work and other local initiatives to support patients and their safety.
As a network pharmacist it is a requirement that you undertake as part of your role and information about this can be found on the Primary Care Pharmacy Education Pathway website and this course will then allow you to complete your independent prescribing qualification.
